For good and valuable consideration, receipt and other sufficiency of which is hereby acknowledged, I hereby irrevocably confer Megan Denise Webb and/or her assigns, successors, agents, and designates the right to copyright, publish, and use in all forms and media and in all manners, including composite or distorted representations, for advertising, promotion, trade, or any other lawful purpose still, single, multiple, or moving photographic portraits or pictures of the following property that I own or have sole authority over to license for the taking of photographic portraits or pictures.

Such photographs may be used in conjunction with my own or a fictitious name, and reproductions thereof may be made in color or otherwise or in the form of derivative works made through and medium.  I waive any right to inspect or approve the finished version(s) or the advertising or other cop that may be used in connection therewith or the use to which it may be applied.

I release and agree to hold harmless Megan Denise Webb and/or her successors, agents, and designates from and liability resulting from such use, including but not limited to claims for invasion of privacy, violation of right of publicity, libel, and trademark and copyright infringement.

I have read this release fully and understand its contents.  This release will be binding upon me, my heirs, legal representatives, and assigns.

I hereby affirm that I am of full age and have the right to contract in my own name.
